Field Trip Request Form

Please complete the below form to request permission for your class to attend a field trip.  All Field Trips must be approved before students can leave campus.  If approved, ALL STUDENTS must have a completed signed Field Trip Permission form in order to attend the field trip.  You are responsible to ensure that a copy of all student permission forms are submitted to administration prior to field trip departure.

No unauthorized Field Trips are allowed.
